Send a Care Plan

You can quickly setup a Care Plan to provide your patient with the medications, activities and measurement you would like them to follow. You can also add instructions and document that your patient will always be able to reference in their MedHelper app. Follow the steps below to setup a Care Plan:

• Connect to the MedHelper for Professionals platform
• Click on the Invite a Patient tab
• Populate the Patient's Contact Information
  • Email (required)
  • Last Name (optional)
  • First Name (optional)
  • Date of Birth (optional)
  • Allergies (optional)
  • Gender (optional)
• Add the required Care Plan items
  • Medication
  • Custom Activity
  • Measurements
• Add any instructions under Additional Information
• Send a document by Attaching a File
• Once the Care Plan is completed click Send
