View Your Notes History

You can easily access your Notes from the Care Plan. Follow the steps below:

• Login to your My Active Health app
• Tap the Care Plan tab
• Tap the History button (located in the top right under the + menu)
• You will be displayed the last 90 Days of your Notes

You can also view your Notes in your Report. Click here to see how to access your Report.
