Export Your Care Plan Data

You can export your Care Plan items and the event history to a PDF that can be emailed or printed. Follow the steps below:

• Login to your My Active Health app
• Tap the Dashboard tab
• Tap the Export button
• Tap Export to PDF
• Tap See Your Report
• You will be displayed a report of your Care Plan data
• Tap the Share icon (located at the bottom of the report)
• You can send the report via email or text (a link will be included to view the report). You can also save the report on your device to access it later.
